In order to assist family with legal matters that may occur as a result of the death of a U.S. citizen in Ukraine, a Consular Report of Death Abroad (CRDA) is prepared by the Embassy as soon as we have all necessary documents and information. This document is in English and is based upon the Ukrainian death certificate and the cause of death certificate.
In order to apply for the CRDA, the deceased’s Next of Kin should provide the following documents and information:
- U.S. passport of the deceased;
- Social Security Number of the deceased;
- Affidavit of Surviving Spouse or Next of Kin (PDF 185 KB)
- a copy of the Next of Kin’s passport;
- Ukrainian death certificate (original) with English translation;
- Ukrainian cause of death certificate (original) with English translation;
- the deceased’s last address in the U.S.;
- the address where the deceased stayed in Ukraine;
- whether the burial will take place in Ukraine or in the United States;
- whether the family wants cremation; and
- whether the family authorizes an autopsy.
Certified copies of the CRDA will be sent to the legal Next of Kin and the original will be sent to the Department of State for permanent filing. Up to twenty (20) copies of the CRDA, issued at the time of death, are provided to the deceased’s Next of Kin free of charge. If in the future additional copies are needed, they can be obtained for a fee of US$50. Once an additional copy is needed, please send a check or money order made payable to the Department of State, along with your written request, to the following address:
