Message for U.S. Citizens: Consular visit to Lviv on June 7, 2018

Event:  A U.S. consular officer will visit Lviv to provide routine consular services to U.S. citizens in Lviv and the surrounding areas on Thursday, June 7, 2018 from 12:30 p.m. to 1:30 p.m.

The consular officer will be able to provide limited citizen services on-site.  These include notary services and acceptance of passport and Consular Report of Birth Abroad applications. In addition to any required documents listed on our website, please make sure you have photocopies of all your originals and translations with you. If you need a passport for someone under the age of 16, please understand that in almost every circumstance both parents and the child must come in person to submit the application.  Please note that all of the fees must be paid in exact change in U.S. dollars only.

Note! Effective April 2, 2018, the passport execution fee increased from $25 to $35. The $10 execution fee increase applies to U.S. passport applicants using the DS-11 form, including  first-time applicants over age 16, children under age 16, and applicants who reapply after reporting their previous passport lost or stolen. Accordingly, the total passport fee for such applicants will increase by $10. The $10 fee increase does not apply to adults eligible to renew their passport using the DS-82 form. The Department of State published a Final Rule confirming this fee change on January 31, 2018.

Location: “Swiss Hotel” lobby, Lviv, Ukraine. “Swiss Hotel” is located at 20 Kniazia Romana St. in Lviv.

Actions to Take: All services will be provided by appointment only. To make an appointment, please email your full name, passport number, date of birth and type of service you are requesting to before 3:00 p.m. Tuesday, June 5, 2018.  You can get in touch with us using the same email address or by calling +380-44-521-5566 if you have any questions about any of the services that will be available.
